Alameda’s Youth Transition Partnership: Creating Pathways to Services

In 2013, Alameda County was awarded a 2 year planning grant to determine the causes of homelessness among youth with foster care histories (Youth At-Risk of Homelessness grant). Often recognized as a service and provider rich community, Alameda County began an intentional process to understand the service array in the community for transition age youth. The goal was to understand where youth could access services and what providers were available to assist. Join us for an overview of their process and what Alameda learned from their interviews with youth, community members, and providers. The results of this work now informs their Youth Transitions Partnership intervention model.
